Sports Physical

  1. Print the WestConn physical form (below).
     

  2. Physical MUST be dated ON OR AFTER FEB 15th, 2025
    - Make sure there are dates on both sides
     

  3. The Physical form MUST BE SIGNED by an M.D. or D.O. Physician Assistant or Nurse signatures are not acceptable.
     

  4. Upload to Sportsware.
